Course Change

Submit a COURSE CHANGE Proposal form to:

  1. CHANGE an existing course:
    Course title
    Course number
    Add or remove a Cross-listing
    Number of credits
    Method of instruction
    Course description
    Grading method
  2. Propose existing course to qualify in GENERAL EDUCATION PROGRAM
  3. Propose change to course in GENERAL EDUCATION PROGRAM
  4. Add ONLINE VERSION of an existing, face-to-face course
  5. DELETE an existing course

Please follow the instructions on the form carefully and completely and include:

  • If applicable, existing Syllabus, with proposed changes indicated in Track Changes
  • If applicable, proposed Curriculum Sheet/Academic Map with changes indicated in Track Changes
  • If applicable, email acknowledgement(s) from other department(s)/college(s) (re: cross-listing or overlap of content)
  • If applicable, ONLINE COURSE Proposal Supplement Form for ONLINE VERSION of face-to-face course with Syllabus of existing face-to-face course and Syllabus for ONLINE version
  • If applicable, GENERAL EDUCATION COURSE Proposal Form and Student Learning Outcome templates for existing course to qualify in GENERAL EDUCATION PROGRAM

Proposals must be approved by:

  • Chair, Department/Program
  • Chair, College Curriculum Committee
  • Dean, College

Proposals are forwarded electronically to the Faculty Senate Office from the college curriculum committee (through SAKAI).  The Faculty Senate Office will forward them to the Curricular Affairs Committee (CAC) or Graduate Council.  The Faculty Senate Office will accept only electronic proposals with digital signatures.

If proposed changes to an existing course significantly impact course content and changes are proposed to the level (e.g., 200 to 300), title and description, or credits, or method of instruction, consider creating a NEW COURSE and submit a NEW COURSE Proposal Form.
