Course Change, Add Online, or Deletion

Submit a COURSE CHANGE Proposal form to propose:

  1. CHANGE of an existing course:
    Course title
    Course Number
    Number of credits
    Method of instruction
    Pre-requisite
    Course description
    Grading method
  2. Add ONLINE VERSION* of an existing, face-to-face course
  3. DELETE an existing course
  4. Propose existing course to qualify in GENERAL EDUCATION PROGRAM

Please follow the instructions on the form carefully and completely and include:

  • Existing Syllabus, if applicable, with proposed changes indicated in Track Changes, or
  • *For ONLINE VERSION of face-to-face course: Syllabus of existing face-to-face course, Syllabus for ONLINE version, and ONLINE SUPPLEMENT form
  •  For existing course to qualify in GENERAL EDUCATION PROGRAM, Student Learning Outcome rubrics (one for each assessed outcome)

Proposals must be approved by:

  • Chair, Department/Program
  • Chair, College Curriculum Committee
  • Dean, College

Proposals are forwarded electronically to the Faculty Senate Office from the college curriculum committee.  The Faculty Senate Office will forward them to the Curricular Affairs Committee (CAC) or Graduate Council.  The Faculty Senate Office will accept only electronic proposals with digital signatures.

If proposed changes to an existing course significantly impact course content and changes are proposed to the level (e.g., 200 to 300), title and description, or credits, or method of instruction, consider creating a NEW COURSE and submit a NEW COURSE Proposal Form.
