Basic Life Insurance and Accidental Death and Dismemberment Insurance (AD&D)
The premium rate for basic life insurance is based on your age and contract salary.
See examples below for an employee whose contract salary is $30,000.

Find your age group and multiply rate by your contract salary. this premium payment is in addition to the premium you pay for the Basic coverage Example: You are 33, and you work 20 pay periods per year, your contract salary is $30,000 x .04860 = $1.458 per pay period = $29.16 a year |
