“Delivering Medication Therapy Management Services”
Delivering Medication Therapy Management (MTM) Services is an innovative and interactive training program that explores the pharmacist’s role in providing MTM services to patients. This activity teaches pharmacists the essential skills necessary to become a successful MTM practitioner. The certificate training program will enhance pharmacists’ clinical expertise in evaluating complicated medication regimens, identifying medication-related problems, and making recommendations to patients, caregivers, and other health care professionals.
Goals of this practice-based program:

  • Advance public health and patient care through improved medication use.
  • Provide training to enhance pharmacists’ ability to effectively provide MTM services.
  • Motivate increased numbers of pharmacists to establish MTM services.
  • Communicate benchmark practices for providing MTM services.

Preceptor Training and Development Workshop

Using the DISC Assessment to Understand Human Behavior and Effective Communication
This interactive, engaging session will provide the participant an understanding of the DISC Assessment and the four behavior styles associated with DISC: Dominant, Influence, Steady and Conscientious. Each participant will receive a personal DISC Assessment to assist with their understanding of what DISC is and how it can assist with effectively communicating and working in teams.
What is DISC?
The DISC Assessment gives us valuable information on how we prefer to communicate with others. It additionally, provides insightful instructions on treating and communicating with others the way they want to be treated.
The DISC assessment is a simple, practical and highly accurate tool that measures behaviors, observable indicators. It first helps us to focus on understanding our own communication styles, and then equally as important, how to enhance communications with others. It does not measure intelligence, values, or performance, but rather encourages us to explore all behavior styles and appreciate the differences.
Goals:  Through an understanding of one’s own communication style, enable the preceptor participant  to communicate with the medical team, including students on pharmacy rotations, in an effective manner.
