Unanticipated Problems
These events or unexpected problems/outcomes should be reported on Appendix S – Event Reporting. This form is located on the Research Resources or on IRBNet under “Library Manager.”

Report a Concern
Faculty, staff and students who conduct research on behalf of the University assume responsibility for conducting their duties within the law and in keeping with URI’s ethical standards. In keeping with this responsibility, each individual must:

  • Understand and follow the institutional principles of conduct in the comportment of their University obligations,
  • Ensure that they and the staff who report to them are aware of and operate in accordance with applicable laws and policies, and
  • Report concerns or incidents of non-compliance

If you have questions, concerns, complaints, allegations of undue influence, allegations or findings of non-compliance, or input regarding the IRB that cannot be addressed by contacting the Office of Research Integrity, concerns should be brought to the attention of the Vice President for Research and Economic Development (401-874-4576). Reports can also be made anonymously through the independently administered, URI Ethics Hotline website, or the toll-free hotline at 1-855-222-0764. Please note that there may be a time lag in reporting using this service.
