Forms For Summer Camp


Click on the appropriate links below then print, complete the forms, sign, and BRING WITH YOU to Camp Check-In.

On the first day of camp when checking in and dropping off your camper you will need to have the following forms:

  • Health History
  • Medical Exam (not required for Day Camp)
  • Behavior Agreement
  • Alton Jones Release Form (or Parent/Guardian Agreement)
  • Required Waivers for your Program

For special diets, food allergies, or medical requirements such as special treatments, injections, or if your child is immune compromised, has mobility limitations (e.g. crutches or wheelchair), no spleen, or other special issues, contact the nurse at least two weeks before the program (401-874-8148 or email altonjones@uri.edu).

A Registered Nurse is on site part-time every day and on-call 24 hours each day.  A student nurse will be on site 24 hours each day.  Kent County Hospital in Warwick, RI and Hasbro Children’s Hospital in Providence, RI serve the Camps.  All Counselors are trained in CPR and First Aid.  You will be notified if your child develops any health concerns that require further assessment and treatment by a physician.  You will also be notified if rescue is called for your child or if your child finds an embedded tick.
