These are unofficial reports and may be submitted anonymously.  It should be noted that this form does not replace the two URI Human Resources incident\/injury reporting forms (24 Hour Notification and Incident\/Injury Report) that must be submitted if there is a personal injury involved in the incident.  These two forms are located at  https:\/\/web.uri.edu\/hr\/forms\/ (in the Workers Compensation section).  At GSO, these forms are submitted to Human Resources via the GSO Dean&#8217;s Office. Both of these forms are for URI Staff.\r\nURI students should report illness and injuries to URI Health Services, located in the Potter Building, at 6 Butterfield Road.
