This internal form is for university processing of specific payments and is not to be completed by students.<\/p>\n\n<\/div><\/section>\n\n\n

Purpose:<\/strong> <\/h2>\n\n\n\n

Departments wishing to process tuition and fee payments (full or partial) for undergraduate students use the SGA-2U form.  Adjustments to payments are completed through the SGA-1U form.<\/p>\n\n\n\n

Order of Approvals:<\/strong><\/h2>\n\n\n\n
    \n
  1. Department Authorization<\/li>\n\n\n\n
  2. Foundation (Fund 401 only)<\/li>\n\n\n\n
  3. Office of Sponsored Projects (Fund 500, or chartfield string with a project number)<\/li>\n\n\n\n
  4. Enrollment Services<\/li>\n<\/ol>\n\n\n\n

     Instructions:<\/strong> <\/h2>\n\n\n\n

    This form should not be submitted until sufficient funding is in place in the respective budget category.<\/p>\n\n\n\n

    1. Department completes the SGA form with correct student information, fee payment details and Chartfield String, and confirms that funding is available and in correct budget categories.<\/li>\n\n\n\n
    2. Department accesses the SGA template and downloads the file to their computer.\u00a0 Save with the following naming convention:
      -Department Naming Convention:<\/strong>
      Last Name, First Name – Student ID Number (Ex: Hernandez, Julia – 123456789)
      -Student-Athlete Only Naming Convention:<\/strong>
      ATHL sport LastName, FirstName URI-ID-Number term (Ex: ATHL MFB LastName,FirstName 123456789 WinterJ)<\/li>\n\n\n\n
    3. Facilitate Departmental Authorization review, approval, and signature from an authorized signatory. Departments are responsible for ensuring funding is available in the proper category prior to submitting this form.<\/li>\n\n\n\n
    4. Completed forms should be emailed as an attachment to Enrollment Services at esfagradteam1-group@uri.edu<\/a><\/li>\n\n\n\n
    5. Enrollment Services will review the form for completion and will notify the department if any issues exist.<\/li>\n<\/ol>\n\n\n\n
