To get started, there are a few important health requirements all new students must complete. Please finish the items below by <strong>July 10, 2026<\/strong>. No further action unless you receive an email about missing requirements<\/li>\n\n\n\n<li>Out-of-State students (except AK, AR, DC, HI, ME, NH, and VT) \u2013 request vaccination dates online by using the &#8220;Fast and Easy&#8221; button on the <a href=\"https:\/\/uri.medicatconnect.com\/auth.aspx\">Patient Portal.<\/a><\/li>\n\n\n\n<li>International and Out-of-State students who received their vaccinations in AK, AR, DC, HI, ME, NH, and VT \u2013 must manually enter dates<\/li>\n<\/ul>\n\n\n\n<p><strong>Step 4: <strong>Upload Documents (if applicable) under the <em>Uploads<\/em> tab.<\/strong><\/strong><\/p>\n\n\n\n<p>Upload a scanned copy of the following forms\/items or take a photo from your mobile device and upload the photo to the patient portal:<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>TB\/Tuberculosis Risk Assessment Form (NOTE: This is only required based on answers to the TB Screening Form &#8211; see Step 5)<\/li>\n\n\n\n<li>Photograph of all medications you currently take<\/li>\n\n\n\n<li>Physical Examination Form (recommended if completed in the last 18 months)<\/li>\n\n\n\n<li>Immunization Record&nbsp;&#8211; <strong>Only if you had to enter your vaccination dates<\/strong><\/li>\n<\/ul>\n\n\n\n<p><strong>Step 5: <strong><strong>Complete <u>Required<\/u> Forms under the <em>Forms<\/em> tab.<\/strong><\/strong><\/strong><\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>TB Screening Questionnaire (if you answer YES to any of the questions, you must take the&nbsp;<a href=\"https:\/\/web.uri.edu\/healthservices\/wp-content\/uploads\/sites\/1769\/TB-Risk-Assessment-Fall-2026.pdf\">TB Risk Assessment Form<\/a>&nbsp;to your medical provider, have it completed and upload it under the Upload section) <strong>* Required<\/strong><\/li>\n\n\n\n<li>Health History for New Students (Not Physicals) <strong>* Required<\/strong><\/li>\n\n\n\n<li>Student &amp; Emergency Contact Form <strong>* Required<\/strong><\/li>\n\n\n\n<li>Texting Opt-in\/Opt-out Form (We hope you will opt-in for appointment reminders and any missing requirements) <strong>*Required<\/strong><\/li>\n<\/ul>\n\n\n\n<p><strong>Step 6: <strong><strong><strong>Insurance Card Information under the <em>Insurance<\/em> tab.<\/strong><\/strong><\/strong><\/strong><\/p>\n\n\n\n<p>Follow the instructions under the <em>Insurance<\/em> tab on the Patient Portal to add your health insurance requirement information.<\/p>\n\n\n\n<p><strong><strong>How to Check Your Status<\/strong><\/strong><\/p>\n\n\n\n<p>Please do <strong>not call<\/strong> Health Services to confirm completion. Instead, watch for emails from: <a href=\"mailto:health@uri.edu\">health@uri.edu<\/a> or <a href=\"mailto:noreply@ahpcare.com\">noreply@ahpcare.com<\/a> or secure messages in the Patient Portal.&nbsp; These email notifications will let you know if you\u2019re all set\u2014or what\u2019s still missing.&nbsp;<\/p>\n\n\n\n<p>* Failure to submit this required health information by the deadline may result in a hold on your account that could hinder your ability to register (including add\/drop) for classes.
