The home-study portion of the Pharmacy-Based Immunization Delivery certificate training program is approved for 12 contact hours (1.2 CEUs) of continuing pharmacy education credit (UAN\u00a00202-9999-17-160-H06-P).\u00a0The live training seminar is approved for 8 contact hours (0.8 CEU) of continuing pharmacy education credit (UAN\u00a00202-9999-17-161-L06-P).<br \/>\n<strong>Initial Release Date:<\/strong>\u00a0July\u00a015, 2017<br \/>\n<strong>Expiration date<\/strong>: July 15, 2020 &#8211; PLEASE NOTE: \u00a0NO Home Study credit granted after this date; Live Credit can only be granted within 60 days from the day of the seminar attended.<\/p>\n<h3>Activity Completion Requirements<\/h3>\n<p>To obtain credit for\u00a0<strong>&#8220;Pharmacy-Based Immunization Delivery&#8221;<\/strong>\u00a0complete home study activities, complete both home study and final\u00a0assessments with a grade of 70% or better, attend the live seminar, successfully complete the injection technique assessment, complete the evaluation, and CLAIM credit. <em>To obtain credit, the Learner must CLAIM credit once each component is completed.<\/em> Once credit is claimed, Transcripts of CPE will be available online within 24 hours on the Learner\u2019s CPE Monitor profile at\u00a0<a href=\"http:\/\/www.nabp.net\/\">www.nabp.net<\/a>.\u00a0 The Certificate of Achievement will be available online upon successful completion of the necessary activity requirements on the participant\u2019s \u201cMy Training\u201d page on\u00a0<a href=\"http:\/\/www.pharmacist.com\/\">www.pharmacist.com.<\/a><br \/>\n<strong>Pharmacy-Based Immunization Delivery: A Certificate Training Program for Pharmacists <\/strong>was developed by the American Pharmacists Association.<br \/>\n<strong>Technology Requirements\u00a0 &#8211;\u00a0 <\/strong>Computer and Internet access is required to complete this activity. \u00a0Please visit APhA\u2019s website\u00a0to view the\u00a0<a href=\"http:\/\/elearning.pharmacist.com\/technology-troubleshooting\">Technology System Requirements<\/a>\u00a0to have a positive learning experience.<\/p>\n<h3>Registration and Refunds<\/h3>\n<p>This program is open to licensed pharmacists and pharmacy students only. Program registration is limited to the first 50 participants. NOTE: Please be advised that this program is subject to postponement or cancellation if the required minimum number of registrants is not met at least 21 days prior to the program.<br \/>\nEarly Registration fee: $200 includes continental breakfast, lunch and breaks. Registration fee after January 1 is $225.\u00a0Registration must be completed <a href=\"https:\/\/www.uripharmacycpd.org\/user\/login\">online<\/a> using a credit card or electronic check as payment. Registrants will receive an immediate email registration confirmation as well as a follow up email with instructions for accessing the online self-study component of the program. The instructional email will be sent 1-2 business days after receipt of a paid registration. No refunds for registration cancellations on or after January 1, 2018. Cancellations prior to January 1, 2018 will receive a refund minus a $50 administrative fee provided the registrant has not redeemed the online access code for the self-study component. <strong>Participants who redeem an enrollment code will not receive a refund regardless whether or not they complete any portion of the self-study but will be able to transfer registration to another program<\/strong>.
