Rent Equipment \u2013Insurance Certificate Request<\/p>\n<ul>\n<li>Form to be completed for all 3rd party equipment rental<\/li>\n<li>URI will insure the value of the equipment to be rented<\/li>\n<li>Form is completed electronically online at: <a href=\"https:\/\/web.uri.edu\/riskmanagement\/\">https:\/\/web.uri.edu\/riskmanagement\/<\/a><\/li>\n<li>Certificate requests ~ Leased Equipment \/ Property Form<\/li>\n<li>Form automatically sent electronically to Risk Management<\/li>\n<\/ul>\n<p>3. Property Damage Report Form<\/p>\n<ul>\n<li>Complete this form in the event there is damage to the rented\/leased equipment.<\/li>\n<li>Immediately forward the form electronically to Risk Management to process a property damage claim. cynthia_stanton@mail.uri.edu<\/li>\n<li>It is the student\u2019s responsibility to protect the equipment so that no further damage occurs.<\/li>\n<li>Property Damage Form<\/li>\n<\/ul>\n<p>4. Actors\/Participants \u2013 Release &amp; Assumption of Risk Form<\/p>\n<ul>\n<li>Form is a waiver for all actors &amp; film participants.<\/li>\n<li>Release &amp; Acceptance to film participants.<\/li>\n<li>Participant releases all rights to the film.<\/li>\n<li>Instructor \/ Filming Student retains this form<\/li>\n<li>Release\/Assumption of Risk Form<\/li>\n<\/ul>\n<p>5. Location Use Agreement<\/p>\n<ul>\n<li>If filming on \u201cothers\u201d property \u2013 this form must be completed<\/li>\n<li>Gives URI and film participants rights to film<\/li>\n<li>Explains ownership of the film product<\/li>\n<li>Electronic copy to Risk Management along with the Film Request Form<\/li>\n<li><a href=\"https:\/\/web.uri.edu\/riskmanagement\/wp-content\/uploads\/sites\/2046\/Location-Use-Agreement.pdf\">Location Use Agreement<\/a><\/li>\n<\/ul>\n","protected":false},"excerpt":{"rendered":"<p>1.
