{"id":688,"date":"2014-12-22T13:29:35","date_gmt":"2014-12-22T18:29:35","guid":{"rendered":"https:\/\/web.uri.edu\/riskmanagement\/?page_id=688"},"modified":"2023-10-20T11:43:46","modified_gmt":"2023-10-20T15:43:46","slug":"film-media-student-forms","status":"publish","type":"page","link":"https:\/\/web.uri.edu\/riskmanagement\/film-media-student-forms\/","title":{"rendered":"Film\/Media Student Forms"},"content":{"rendered":"\n<p>1. <a title=\"Filming Request Form\" href=\"https:\/\/web.uri.edu\/riskmanagement\/filming-request-form\/\">Filming Request Form<\/a><\/p>\n\n\n\n<ul class=\"wp-block-list\"><li>To be completed for all student film projects by the student.<\/li><li>Complete online form which is automatically sent electronically to Risk Management.<\/li><\/ul>\n\n\n\n<p>2. <a title=\"CERTIFICATE OF INSURANCE REQUEST \u2013 LEASED EQUIPMENT\/PROPERTY\" href=\"https:\/\/web.uri.edu\/riskmanagement\/coi-request-leased-equipment-or-property\/\">Rent Equipment \u2013 Insurance Certificate Request<\/a><\/p>\n\n\n\n<ul class=\"wp-block-list\"><li>Form to be completed for all 3rd party equipment rental.<\/li><li>URI will insure the value of the equipment to be rented.<\/li><li>Form is completed electronically online (link above)<\/li><li>Form automatically sent electronically to Risk Management.<\/li><\/ul>\n\n\n\n<p>3. <a href=\"https:\/\/web.uri.edu\/riskmanagement-draft\/wp-content\/uploads\/sites\/2046\/Bodily-Injury_Property-Damage-Report-Non-Student-or-Staff-9-11-14.pdf\">Property Damage Report Form<\/a><\/p>\n\n\n\n<ul class=\"wp-block-list\"><li>Complete this form in the event there is damage to the rented\/leased equipment.<\/li><li>Immediately forward the form electronically to Risk Management to process a property damage claim.  <\/li><li>It is the student\u2019s responsibility to protect the equipment so that no further damage occurs.<\/li><\/ul>\n\n\n\n<p>4. <a href=\"https:\/\/web.uri.edu\/riskmanagement-draft\/wp-content\/uploads\/sites\/2046\/Actor-Crew-Release-Form-12-26-14.pdf\">Actors\/Participants \u2013 Release &amp; Assumption of Risk Form<\/a><\/p>\n\n\n\n<ul class=\"wp-block-list\"><li>Form is a waiver for all actors &amp; film participants.<\/li><li>Release &amp; Acceptance to film participants.<\/li><li>Participant releases all rights to the film.<\/li><li>Instructor \/ Filming Student retains this form.<\/li><\/ul>\n\n\n\n<p>5. <a href=\"https:\/\/web.uri.edu\/riskmanagement-draft\/wp-content\/uploads\/sites\/2046\/Location-Use-AGreement-12-22-14.pdf\">Location Use Agreement<\/a><\/p>\n\n\n\n<ul class=\"wp-block-list\"><li>If filming on \u201cothers\u201d property \u2013 this form must be completed.<\/li><li>Gives URI and film participants rights to film.<\/li><li>Explains ownership of the film product.<\/li><li>Electronic copy to Risk Management along with the Film Request Form.<\/li><\/ul>\n","protected":false},"excerpt":{"rendered":"<p>1.
