Run Reports

URI EMS maintains documentation of each call we respond to. They are available upon request. If you would like to obtain a copy of a run report, the patient who was treated/transported must sign an authorization to release/waiver form (available underDocuments/Forms). Substitute forms are not acceptable. Please fax this form or mail an original copy. As URI EMS has no full-time office staff, please allow 7-14 days for processing. A $10 processing fee will apply. Please make check or money order payable to “URI EMS c/o URI Foundation Acct # 5321”. Please do not send cash.

In compliance with federal standards including HIPAA (Health Information Portability and Accountability Act), URI EMS maintains confidentiality of all protected patient healthcare information. For more information, please feel free to contact us.

For inquiries regarding anything else, please Contact Us or the URI Police Department(dispatch logs). The URI News Bureau may also be able to assist.
