Starting Monday, July 27, all faculty, staff, students, and visitors coming to any University of Rhode Island campus will be required to complete a daily health self-assessment electronic questionnaire.
Daily monitoring of one’s health and well-being prior to coming to campus/work can aid in early detection of infectious disease and is an effective measure to prevent community spread of COVID-19. In our ongoing effort to protect our on-site community, we have established this interim daily health self-assessment for our community and visitors to complete each day they plan to physically come to any of the URI campuses or leave their residence halls to start their day.

Employees are still required to have approval from their supervisors to work on campus. Once an employee has completed the short questionnaire, they will receive an email confirming their response evaluation. If you have been approved to come to campus by your supervisor and your self-assessment supports your return to campus, be prepared to show that day’s email confirmation upon request by your supervisor or manager. All supervisors and managers have the responsibility to request a copy of or to see an employee’s response email any time they are on campus.
Those employees who can work remotely to fulfill some or all of their work responsibilities may continue to do so to reduce the number of individuals on campus this summer and the potential spread of the COVID-19 virus.
Any students returning to URI campuses should be prepared to show their daily email confirmation upon request. All visitors to URI campuses will be asked to present the email confirmation upon arrival at campus.
This health self-assessment will be part of our daily routines prior to coming to any URI campuses for the foreseeable future. The University is developing a smartphone app that will replace this interim daily health assessment form. It is anticipated the app will be ready prior to the fall semester. Updates will be provided to the URI community regarding the change from this form to the new app.
In the coming weeks the University will finalize its carefully designed and robust COVID-19 testing plan to monitor the health of our community and minimize the likelihood of outbreaks. The University is collaborating with the Rhode Island Department of Health to finalize testing protocols and procure testing equipment and supplies. We look forward to sharing the testing plans in the coming weeks.
