PLEASE READ WHAT YOU ARE SIGNING!
1) AUTHORIZATION:
All current Harrington community members will be granted the ability to reserve equipment upon completion of this agreement.
Select Harrington courses will be granted access to additional equipment and facilities as determined by department faculty.
2) RESERVATIONS:
Equipment is available on a first come, first served basis. The Media Equipment Center is a “Reservation First” operation, meaning all checkouts need to be requested ahead of time. Checkouts without reservations may be accommodated on a limit basis
All reservations* must be submitted through Patron Portal by 6:30pm on the business day prior to checkout. Reservations are accepted up to two weeks in advance of the selected checkout date.
3) EQUIPMENT CHECKOUT:
The Patron listed on this form is the only person allowed to checkout equipment under their name. This means that no one else may pick up any equipment for any Patron but themselves. Patrons may only checkout equipment relevant to their access level and once they have received an in-class demo for course-allocated equipment.
Patrons must inspect all equipment to be checked out of the HUB Media Equipment Center (107 Ranger Hall) before leaving. Inspection includes verifying that the equipment is in good working order and all listed components are present. If something is missing or damaged, notify HUB staff, who will rectify issues on-site.
Upon completion of inspection, Patrons must verified the working condition of the equipment and the inclusion of all component parts, and assume financial responsibility for all damage or loss incurred while said equipment is in their possession. Once Patrons leave the HUB MEC, all loss or damage assessed by HUB staff upon return is the responsibility of the Patron.
Patrons may checkout equipment from the HUB for a limited period of time, and agree to pay late fees upon failure to return equipment before the time specified on their checkout. Limited extensions may be granted on the scheduled return date pending equipment availability. It should not be assumed that all equipment will be available. Traffic, illness, not being on campus that day, and other personal issues are not considered valid excuses for tardiness.
Patrons will receive email notifications regarding the status of reservations, checkouts, returns, and late equipment. It is the responsibility of the Patron to verify the receipt and to monitor these email notifications. If these emails are not being received, it is the responsibility of the Patron to inform HUB staff.
If alternative methods of communication are required it is the responsibility of the Patron to inform HUB staff.
4) USE & CARE:
Patrons are required to immediately notify the MEC of any equipment issues. Patrons should not attempt repairs or disassemble any equipment.
The patron who the equipment is reserved under is solely responsible for loss and/or damage, including any items that were not previously reported as missing or damaged at the time of checkout.
5) RETURNING EQUIPMENT & LATE FEES:
Patrons must return equipment to the HUB Media Equipment Center (107 Ranger Hall) by the time it is due as stated by HUB staff, IN THE SAME CONDITION IT WAS CHECKED OUT IN.
Returns occur in two phases; an initial examination and a more thorough Quality Control Process. Issues that are not found during the initial check may be found during quality control.
Late fees are incurred when equipment is brought back after the time specified on their checkout. Once this time has passed, fines will be assessed by HUB staff according to the following schedule:
Equipment Late Fees – $5 per piece of equipment, per day late.
Lost Printed Component Cards – $5 per card
Patrons with unresolved late fees cannot use the HUB until the fee is paid. Equipment more than one-week late will be reported to campus police as theft.
6) DAMAGE / LOSS / IMPROPER USAGE:
Patrons are responsible for damaged or lost equipment once it is in their possession. If a Patron incurs damage/loss to the equipment, they cannot use the HUB until a fee has been paid equal to the repair/replacement cost of the item.
Unresolved damage/loss fees may result in a registration hold for the Patron, and may be reported to campus police. The HUB reserves the right to terminate or suspend access at any time for violations of policy and any damages to or improper usage of equipment.
