Student Conduct Record Requests

Student Conduct Records are considered private records and, therefore, may only be released to a third party with the permission of the student. Submitting a conduct record request form means authorizing the University to send your student conduct record directly to a third party. 

If there is a form to be completed and sent to a third party, you must complete the authorization section and upload it to us.

Information About What is Reported: Reporting is limited to conduct cases with a finding of “responsible”. Cases with a finding of “not responsible” and warnings are not reported. Pending conduct cases, if applicable, will not be released as no determination has been made regarding responsibility, and the student is entitled to full due process before it is included in this type of release. Conduct cases are maintained by the Office of Community Standards for a limited period as outlined in the Student Handbook, typically 6-7 years after the date of the incident. Incidents occurring outside of the defined period may not be reported.

Students should be aware that in accordance with federal law, the University may, without consent, release information from conduct records if the record contains violations involving violence or behavior related to safety and well-being.

PLEASE NOTE: We will work diligently to complete this request in a timely manner. However, due to the volume at any given time, the soonest we can guarantee completion is ten (10) business days from receipt of completed application materials.
