Students must take action by August 16: Upload proof of COVID-19 vaccination or request exemption

As we enter August and inch closer to the beginning of the fall semester, we hope all of our students are taking time out to recharge and enjoy the remaining days of summer. Thank you to the more than 8,200 students so far who have logged on to the URI Health Services Patient Portal and provided their proof of COVID-19 vaccination or applied for an exemption. We look forward to welcoming you back to campus soon.

As you may recall, earlier this summer, the University announced that it would require all graduate and undergraduate students returning to campus in the fall be vaccinated against the COVID-19 virus or have an approved exemption. Students must provide proof of that vaccination or apply for an exemption by August 16 to avoid a COVID-19 Hold being placed on their accounts. (Directions provided below.)

With the proliferation of the more contagious COVID-19 Delta variant, it is critical that all those who are able receive the vaccine to protect the health and safety of our community. COVID‑19 vaccines are widely available, safe, limit the transmission of the virus, and have been shown to nearly eliminate the chances of death or serious illness related to a COVID‑19 infection.

If you are unsure of where to obtain a vaccine in your area, contact your local health department or visit Vaccines.gov. If you are in Rhode Island, URI will be holding Vaccine Clinics on its Kingston Campus throughout the month of August and into September. If you have not yet been vaccinated, we would encourage you to register for one of these clinics. If you have concerns about obtaining a COVID-19 vaccination, please consult with your health care provider.

If you still need to submit your full proof of vaccination:

  • Go to uri.medicatconnect.com
  • Select ‘I have a uri.edu email address’ to sign in with your single sign on credentials (Microsoft Office365 login)
  • Select ‘Immunizations’
  • Enter your COVID-19 vaccination information and hit submit
  • Select the ‘Uploads’ tab
  • Select ‘COVID-19 Vaccination Card’ from the ‘Choose Document to Upload’ menu
  • Upload a photo or scanned copy of your vaccination card

If your vaccine requires two doses, you should upload a copy of your vaccination card after each dose.

If you wish to request a vaccination exemption:

  • Go to uri.medicatconnect.com
  • Select ‘I have a uri.edu email address’ to sign in with your single sign on credentials (Microsoft Office365 login)
  • Print the exemption form you wish to complete, then complete and sign the form
  • Select the ‘Uploads’ tab
  • From the ‘Choose Document to Upload’ menu, select COVID-19 Vaccine Medical OR Religious Exemption Form and upload the form from a saved file or by taking a photo of it

Students who do not provide full proof of vaccination or request an exemption before August 16, will have a COVID-19 Vaccine Hold placed on their accounts and will be unable to access in-person classes, housing, and other on-campus services. If you are arriving to campus early, you must be compliant prior to arrival. COVID-19 Vaccine Holds will be removed 5 to 7 business days after proof of vaccination is uploaded or an exemption form is approved.
