Incident Reporting Form

Staff and students should call 911 for all serious and life threatening accidents and injuries. This form is used by the EHS office to evaluate safety areas of concern, focus training efforts and share lessons learned. These are unofficial reports and may be submitted anonymously. It should be noted that this form does not replace the two URI Human Resources incident/injury reporting forms (24 Hour Notification and Incident/Injury Report) that must be submitted if there is a personal injury involved in the incident. These two forms are located at https://web.uri.edu/hr/forms/ (in the Workers Compensation section). At GSO, these forms are submitted to Human Resources via the GSO Dean’s Office. Both of these forms are for URI Staff. URI students should report illness and injuries to URI Health Services, located in the Potter Building, at 6 Butterfield Road.
