Run Report Requests

URI EMS documents every emergency call we respond to, and records are available upon request. To obtain a copy of a run report, the patient who received treatment or transport must complete and sign the Authorization to Release/Waiver Form (available under Documents/Forms). Substitute forms are not accepted.

Please submit the completed form by fax or mail an original copy. As URI EMS does not have full-time office staff, please allow 7–14 days for processing. A $10 processing fee applies—payments must be made by check or money order payable to:
