Overview

“Caring for the Complex Patient: Managing Patients with Diabetes, Heart Failure, and Chronic Kidney Disease,” focuses on gaps in medical treatment for patients with certain complex conditions, barriers to guideline directed medical therapy (GDMT), and best practices to optimize GDMT. 

Target Audience

Pharmacists in all practice settings, and pharmacy technicians.

Learning Objectives

Upon completion of this educational activity, pharmacists will be able to:

  • Explain how to assess for gaps in medical treatment for the complex patient with Diabetes, Heart Failure and Chronic Kidney Disease
  • Describe barriers which preclude patients from achieving Guideline directed medical therapy (GDMT) such as socioeconomic, adverse effects, pill burden etc.
  • Outline how to develop a plan to optimize medication therapy, weighing patient-specific risks and benefits.
  • Review the emerging role of SGLT-2 inhibitors for use in heart failure beyond comorbid diabetes management.
  • Discuss best practices to optimize guideline directed therapy for heart failure patients across the care continuum.

Upon completion of this educational activity, pharmacy technicians will be able to:

  • Identify complex patients with Diabetes, Heart Failure and Chronic Kidney disease based on review of a medication list.
  • Identify select antihyperglycemic agents newly being prescribed for patients without diabetes mellitus type 2.

Evaluation

All participants will have the opportunity to review the educational sessions and speakers and to identify future educational needs.
