Reporting Concerns Regarding Human Subjects
Unanticipated Problems
Adverse events that arise during the course of a project involving risks to human subjects or others must be reported immediately to the IRB. These events or unexpected problems/outcomes should be reported on Appendix S – Event Reporting.
Report a Concern
Faculty, staff, and students who conduct research on behalf of the University assume responsibility for conducting their duties within the law and in keeping with URI’s ethical standards. In keeping with this responsibility, each individual must:
- Understand and follow the institutional principles of conduct in the comportment of their University obligations,
- Ensure that they and the staff who report to them are aware of and operate in accordance with applicable laws and policies, and
- Report concerns or incidents of non-compliance
If you have questions, concerns, complaints, allegations of undue influence, allegations or findings of non-compliance, or input regarding the IRB that cannot be addressed by contacting the Office of Research Integrity, concerns should be brought to the attention of the Vice President for Research and Economic Development 401.874.4576.
Reports can also be made anonymously through the URI Ethics Hotline, an independently administered, toll-free hotline at 1.855.236.1845 or https://uri.alertline.com/gcs/welcome. Please note that there may be a time lag in reporting using this service.
