IACUC Process and Forms

Initial Submission Process

To initiate the IACUC review process, thoroughly complete an application.

For research or teaching activities involving interaction with vertebrate animals, use the IACUC New and Three-Year Renewal Protocol Form and the IACUC Personnel Table.

For research or teaching activities ONLY comprised of observation of vertebrate animals, use the IACUC Observational Studies Protocol Form.

Submit the following via IRBNet:

  • Appropriate application form
  • Personnel table
  • CITI training certificates for all personnel
  • Standard operating procedures (SOPs)
  • Relevant permits, MOUs, and grant materials

Subsequent Submission Process

To request changes to an approved protocol, complete and submit the following via IRBNet:

  • IACUC Protocol Amendment Form
  • IACUC Personnel Table (if applicable)
  • CITI training certificates for new personnel
  • Revised approved Protocol Form with changes tracked
  • New or revised (with changes tracked) study documents (e.g., SOPs, permits, MOUs, grant materials)

All approved protocols must be renewed every three (3) years. Renewals are reviewed as if they are completely new protocols, so follow the Initial Submission Process above to initiate three-year de novo review.

Approved protocols must be terminated upon completion of the project. To terminate a protocol, completed and submit the following via IRBNet:

Review Process

Submissions receive an initial review by ORI personnel, who will reach out to you to request any changes needed to get your submission ready for review by the committee. Depending on the specifics of the project, the submission may be eligible for Administrative Review, Designated Member Review (DMR), or Full Committee Review (FCR).

  • Administrative Reviews and Designated Member Reviews are conducted on a rolling basis throughout the year by a member of the Office of Research Integrity or members of the IACUC.
  • Full Committee Reviews must be conducted at a convened meeting of the IACUC.

After initial review by one of the processes above, most submissions require modifications before receiving full approval. You can accelerate approval of a submission by providing complete and thorough materials and responding rapidly to requested modifications.
