IBC Process and Forms

Initial Submission Process

To initiate the IBC review process, thoroughly complete the IBC New and Five Year Renewal Registration Form and submit the form via IRBNet.

Subsequent Submission Process

To request changes to an approved registration, complete and submit the IBC Amendment Form via IRBNet.

Registrations are valid for five (5) years, at which time a de novo registration must be reviewed and approved. To initiate renewal, complete and submit the IBC New and Five Year Renewal Registration Form via IRBNet at least three (3) weeks before expiration.

Approved Biological Research registrations must be terminated upon completion of the project. To terminate a protocol, completed and submit the IBC Termination Form via IRBNet.

Review Process

Registrations receive an initial review by ORI personnel, who will reach out to you to request any changes needed to get your registration ready for review by the committee. Depending on the specifics of the project and the level of risk, the registration may be eligible for Administrative Review, Expedited Review, or Full Committee Review.

  • Administrative Reviews are conducted on a rolling basis throughout the year by a member of the Office of Research Integrity.
  • Expedited Reviews are conducted on a rolling basis throughout the year by one or more members of the IBC.
  • Full Committee Review must be conducted at a convened meeting of the IBC.

After initial review by one of the processes above, most registrations require modifications before receiving full approval. You can accelerate approval of a registration by providing complete and thorough materials and responding rapidly to requested modifications.
