This form, completed with as much information as possible, is to be provided to the Risk Management Office, no later than 4 business days prior to the start of the rental period.

SPECIAL NOTE REGARDING ENTERPRISE RENTALS: Must be 21+ years of age to drive 12 or 15-passenger vehicles rented from Enterprise. URI Certificates of Insurance (COI) will be processed within 3 business days and will be emailed directly to the University member requesting the certificate.

DAMAGE TO RENTAL VEHICLES MUST BE REPORTED ASAP TO THE RISK MANAGEMENT OFFICE – 401-874-2591
