Conversations & Goals of Care

Many providers do not have the skills to communicate with their patients about what matters most to the patient at end of life. The purpose of this webinar is to enable participants to recognize when their patients are approaching the end of life; to be able to describe the difference between hospice and palliative care and their benefits to patients/families, and to assist their patients in the transition to palliative care and hospice. After completing this webinar, participants will be able to:

  • Identify the difference between hospice and palliative care and their benefits
  • Recognize patients with advancing serious illness
  • Assist patients in the transition to palliative care and hospice
  • Assess resources that may improve quality of care, patient satisfaction and reduce cost
