This form is not to be used by current URI state employees. State employees must contact HR. Current students and student employees use this form to request an e-Campus username change. Your URI email will also be changed to match the new username. Enrollment Services reserves the right not to approve username changes deemed offensive, confusing, misleading, already in use or for other reasons.
