Adverse drug events pose a serious concern in older adults during the post-hospitalization period. This is a time of high-risk for older patients, when multiple medication changes occur that can lead to confusion regarding medication management for both patients and providers.

  2. Identify errors & hazards in care by understanding best practices in prescribing to older adults
