Module 1

Principles of Medication Use in Older Adults

Adverse reactions in everyday primary care practice continue to occur commonly in older adults, especially as patients are taking an ever increasing number of drugs. Older adults continue to be prescribed the “usual” adult dosages of both new and existing drug therapies, despite lacking evidence of their safety and efficacy.

Learning Objectives

After completing this webinar, participants will be able to:

  1. Evaluate the potential effect of physiologic changes on the pharmacokinetics and pharmacodynamics of common medications in healthy & frail older adult.
  2. Apply knowledge of altered pharmacokinetics and pharmacodynamics to the care of older adults.

Module 2

Medication Assessment and Quality Parameters

The interplay between an older patient’s need for medications to manage chronic disease and their increased likelihood to experience an adverse drug event from these medications leaves health care professionals in a difficult situation. Adding to the difficulties in understanding complex medication regimens, health care providers and health care groups/institutions may be held accountable for the use of risky medications that lead to poor patient outcomes.

After completing this webinar, participants will be able to:

  1. evaluate existing medication assessment tools;
  2. understand how to analyze a complex medication regimen for an older person;
  3. explain how medication measures are used to measure quality of care.

Deprescribing:  Implementing “Less is More”

Despite the availability of algorithms and guidelines, clinicians report discomfort with deprescribing in older adults due to concern for withdrawal reactions, lack of self-efficacy, time constraints, lack of clarity regarding the best approach, and unclear assessment of risk versus benefit.

  2. Understand barriers to deprescribing and gain clarity on best approach.
