Core Concepts in Age Friendly Care: Mobility Matters

The goal of this activity is to improve the comfort level and confidence of primary care teams in more comprehensively assessing older adults for functional or cognitive impairment and how to identify frailty and impending serious illness. The session will focus on geriatric assessment as a tool to help primary care teams identify patients with additional needs.

Mobility in Older Adults:  Impact of Covid-19

An abrupt reduction in activity levels, as can happen with social isolation, is of particular concern in older individuals, who are typically more prone to frailty, sarcopenia, and chronic diseases. Considering the impact of abrupt inactivity on overall health, it is essential that healthcare professionals become aware and learn strategies to mitigate the potential negative effects of isolation on this vulnerable population.

While falls are a major cause of morbidity and mortality in older adults, it can be difficult for primary care providers to add comprehensive functional assessments into visits of those patients at risk for functional decline and falls.

Effective Interventions for Enhancing Mobility in Older Adults

With life expectancy continuing to rise there is an increasing public health focus on the maintenance of physical independence among all older adults. Identifying interventions that improve physical outcomes in pre-frail and frail older adults is imperative.

  1. Describe physiological aspects of aging that impact mobility.
  2. Identify evidence-based interventions to improve mobility across the continuum of physical functioning.
  3. Recognize community-based resources currently available to support mobility in older patients.
