Submission of this request does not constitute insurance coverage. Coverage changes are not effective until approved by the appropriate administrative authority, processed by Risk Management, and written confirmation has been provided by the insurance carrier. As a reminder, if it isn't scheduled, it isn't covered.

REGISTRATION REQUIREMENTS

IMPORTANT: Prior to completing this form, approval from either the Graduate School of Oceanography (GSO) Dean or Chief Business Officer (CBO) must be obtained. A copy of the approval email must be attached below before proceeding with the registration request. Requests submitted without approval documentation cannot be processed.

These terms contain important coverage conditions, exclusions, reporting requirements, and insured obligations that must be followed throughout the policy period. If you are unable to comply with a specific term, condition, or obligation, you must identify the issue and obtain written approval for any exception prior to coverage being considered.
