Tuition and Fees Statement (1098-T)

Important Notice:

The 2023 1098-T forms will be issued by Heartland ECSI on or before January 31, 2024.  Students can also access their forms electronically by visiting the Heartland ECSI website. Instructions for online access and answers to frequently asked questions can be found on the Heartland ECSI Tax Documents webpage.

Provide SSN

The University is required to report certain information concerning your status as a student to the Internal Revenue Service (IRS) on a Form 1098-T, Tuition Statement. In order to properly issue your tax statement, a valid Social Security Number (SSN) or Tax Identification Number (TIN) must be provided.

If we do not have a social security number on file for you, there may be a request for this information on your e-campus To-Do List.   To complete the request, simply click on the Update Social Security Number item and follow the prompts.  When complete, the item will no longer appear on your To-Do List, and your social security number will be visible on your Student Center General Information tab.

Please note that your social security or tax identification number will be used only for IRS reporting purposes and will not be used as an identifier on your student record. We encourage all students to verify that their University record contains a valid SSN or TIN. Enrollment Services will remind students of our need for this information via email each year.

If you need to make a correction to the SSN or TIN we have on file for you, please complete and submit form W-9S to Enrollment Services.

International Students
If you are an international student, and there is a request to Update Social Security Number on your e-campus To-Do List,  please click on the item and follow the prompts to complete the request.  You will be given an opportunity to enter your SSN or TIN if you have one, or select the option that indicates you have not been issued an SSN or TIN.  Upon completion, the request will no longer appear on your e-campus To-Do List.
