This portal should only be used when submitting an SGA-2G/1G form and not for SGA-2G forms that are part of the Assistantship Contract packet (reference the Assistantship Contract Online Portal page) 

Please do not submit the forms directly to the approving departments (e.g. Foundation Office or Office of Sponsored Projects), hand-deliver, or send SGA-2G/1G forms through interoffice mail. Once the form is submitted, the approving departments will review the forms based on the funding source.

Rename the SGA Form to the following format:
Last Name, First Name Student ID number
(Example: Smith, Jane 123456789)
