The PLA Permission Form is the first step to requesting a Prior Learning Assessment (PLA) or Challenge Exam. This form must be signed by all of the following: a Faculty Assessor, Department Chair and College Dean.
The PLA Evaluation Form is the last step of a Prior Learning Assessment (PLA) and is attached to the final portfolio at the conclusion of PLA 100: Portfolio Development and upon submission to the Faculty Assessor identified on the PLA Permission Form.
