The PLA Permission Form is the first step to requesting a Prior Learning Assessment (PLA). This form must be signed by a Faculty Assessor, Department Chair and College Dean. Once form is completed, return to the Coordinator for Prior Learning.

The PLA Evaluation Form is the last step of a Prior Learning Assessment (PLA) and is attached to the final portfolio at the conclusion of PLA 100: Portfolio Development and upon submission to the Faculty Assessor identified on the PLA Permission Form.

The CLEP Prior Approval Form is required for any current URI student interested in sitting for a CLEP exam. Prior approval from your College Dean is required. The completed form must be submitted to the Coordinator for Prior Learning prior to taking the exam.

The Proficiency Exam Form is required for any current URI student interested in sitting for a proficiency exam. This form must be signed by the Academic Department in which the exam is to be attempted and your College Dean prior to being returned to Enrollment Services.
