If you plan on completing your CELS Undergraduate Certificate(s) this academic year or have already completed your certificate(s), it is important to submit your completed application. Once your Undergraduate Certificate completion has been confirmed, a signed copy will be emailed to you.

Please be sure to have your Undergraduate Certificate form signed by the appropriate program coordinator or department chair prior to submission.
