- Medication management during care transitions from the inpatient setting to home is a key component in reducing hospital readmissions. Pharmacist involvement during care transitions as part of a multidisciplinary team approach is outlined as a best practice by ASHP and APhA.
- A recent study evaluated the impact of a phone call made by a pharmacist 3 to 7 days postdischarge on hospital readmission. Readmission rates after contact versus no contact were as follows: 0.8% vs. 4% (p=0.01) at 7 days; 5% vs. 9% at 14 days (p=0.04); and 12% vs. 14% (p=0.29) at 30 days, respectively. - In June of 2013, Pharm D candidates from the University of Rhode Island initiated an experiential rotation with the Visiting Nurse Service affiliated with South County Hospital in Washington County, Rhode Island.
