Pharmacy Intern License Information

Starting the first professional year (P1), all students in the professional Doctor of Pharmacy (Pharm.D.) program at the University of Rhode Island College of Pharmacy are required to hold a Rhode Island pharmacy intern registration while enrolled. If your experiential rotations (both introductory and advanced) are in other states, you may also need additional intern licenses. Those applications for additional states should be started as soon as your experiential placements are confirmed as the application processing can take time. No internship hours can be started before any needed intern registration is completed.

Frequent States

Rhode Island – Pharmacy Intern (“Limited”) License 

Note: Required of all Pharm.D. students at the URI College of Pharmacy. 

Key Application Elements

  • Application fee of $70 by check or money order affixed to the top-left of the first page of the application. 
  • Copy of driver’s license or state-issued ID verifying the applicant is at least 18 years of age. 
  • College enrollment certification – page 5 of the application should be submitted to the Associate Dean for signing to confirm active enrollment in an accredited Pharm.D. program.  
  • Intern license is valid for up to five years, provided the student maintains timely progress toward graduation. 

Connecticut – Pharmacy Intern License 

Key Application Elements

  • Application fee of $60. 
  • Intern license is valid while student is enrolled in an accredited pharmacy program

Massachusetts – Pharmacy Intern License 

Key Application Elements

  • One 2”x2” passport-style photo. Note: it is recommended to get a passport photo rather than cropping a regular photo.  
  • Application fee of $95. 
  • Intern license is valid for up to five years, provided the student maintains timely progress toward graduation. 

Maine – Pharmacy Intern/Student Pharmacist License 

Key Application Elements

  • Proof of enrollment/standing documentation should be submitted to the Associate Dean for signing to confirm active enrollment in an accredited Pharm.D. program. This is collected within the online portal. 
  • Intern license must be renewed yearly

New York – Limited (Intern) Permit 

Note: Students will not be eligible to submit a NY intern application until the first professional year (P1) has been completed. 

Key Application Elements

  • Completed Form 5: Application for Limited (Intern) Permit. Section 1 is completed by the student. The affidavit must be signed in front of a notary. Note: The College has two notaries that can be used, Assistant Dean Denise Gorenski and Dawne Strickland. 
  • The completed application and the application fee of $70 should be submitted to the Associate Dean to certify enrollment (check or money order for application fee should be left in the folder outside the Associate Dean’s office). The Associate Dean’s office will send the completed application to the New York Office of the Professions. 
  • Note: Application will not be able to be completed unless student is certified in cardio-pulmonary resuscitation (CPR). 
  • Intern license is valid for up to five years, provided the student maintains timely progress toward graduation. 

New Hampshire – Pharmacy Intern License 

Key Application Elements

  • Proof of enrollment/standing documentation should be submitted to the Associate Dean for signing to confirm active enrollment in an accredited Pharm.D. program. This is collected within the online portal. 
  • Application fee of $25. 
  • Intern license must be renewed yearly. Renewal fee is $25. 

New Jersey – Pharmacy Intern Registration 

Note: Intern registration in New Jersey is only required for those who work in paid positions in New Jersey; they are not required for experiential rotations as part of the curriculum of an accredited Pharm.D. program.
