URI Campus Recreation Request Form
To request a membership or program enrollment refund/transfer, patrons are required to complete the following form. Refunds are granted at the full discretion of the University of Rhode Island Campus Recreation professional staff. Do not leave out any information or we will not be able to process your refund request. Please, attach any relevant forms or documentation (i.e., doctor’s note). For any questions or to check the status of your request, contact campusrec@etal.uri.edu. Please, allow 3-5 business days before contacting to check on the status. Refunds and/or credits on account will not be automatically applied to the member's account in the case of lost time due to unforeseen emergencies, problems, or maintenance, and falls under the discretion of Campus Recreation administration. URI Campus Recreation will do all it can to ensure patrons are aware of unexpected closures and issues impacting facilities and services.
