URI Campus Recreation Request Form

To request a membership or program enrollment refund/transfer, patrons are required to complete the following form. Refunds are granted at the full discretion of the University of Rhode Island Campus Recreation professional staff. Do not leave out any information or we will not be able to process your refund request. Please, attach any relevant forms or documentation (i.e., doctor’s note). For any questions or to check the status of your request, contact campusrec@etal.uri.edu. Please, allow 3-5 business days before contacting to check on the status. Refunds and/or credits on account will not be automatically applied to the member's account in the case of lost time due to unforeseen emergencies, problems, or maintenance, and falls under the discretion of Campus Recreation administration. URI Campus Recreation will do all it can to ensure patrons are aware of unexpected closures and issues impacting facilities and services.

User Information

First and Last Name as shown on your drivers license/state ID/passport. Please, provide the name of the person who paid for the membership/program.
Indicate name of participant if different than the individual submitting request (ie. Participants: Child's Name; Requester: Parent/Guardians Name)

Locker Service

  • All belongings are to be removed from locker at the time of refund request submission. Any and all items not removed from the locker will be removed by Campus Recreation staff and discarded during the next semesterly locker clearance cycle.
  • All approved cancellation request will accrue an administration fee ($10: Semester Locker and $20: Annual Locker) .

Membership Refund/Cancellation Information

  • Day pass sales are non-refundable and cannot be transferred towards the cost of a membership or any other service/program.

  • Approved cancellation request made within the first week of membership purchase (Semester or Annual) will not be subject to a cancellation fee/charge.

  • Semester and Annual Campus Recreation Memberships will be considered for a prorated refund at the discretion of Campus Recreation management staff.

  • Request made for one of the following reasons will require documentation and may not accrue the administrative fee:
    -Moving out of local area (+20 mi/32 km)
    -Formal leave of absence and sabbaticals
    -Changes in medical condition resulting in physical limitations

Note: All refund request not made within the first week of membership purchase are subject to a cancellation fee (Semester Membership Fee: $20 Fee; Annual Membership Fee: $35 for Community, $30 for URI Employee/Affiliate).

Program/Service Area

The operations of each URI Camus Recreation program and service may vary due to the nature of the program and/or service. Please, thoroughly read the cancellation policy for the specified program/service that you are submitting a request for. All refunds and cancellations are granted at the full discretion of University of Rhode Island Campus Recreation professional staff.

Aquatics & Certifications

Are you submitting a request to transfer from one program offering into another OR Cancel your current program registration?

General Aquatics Transfer Fee Policy

  • All withdrawals/refunds/transfers will require an administration fee according to the program transaction.

  • Refund transactions must take place at the Customer Service Center for all customers that registered in person or online and did not save their credit card.

  • Refund transactions can be processed from an administrator's desk if the patron's credit card is saved in Fusion.

  • A doctor's note will waive the administration fee.

  • A death waives the administration fee.

CPR Pro Rescuer and First Aid Transfer and Cancellation Policy 

  • All withdrawals/refunds/transfers for CPR Pro Rescuer and First Aid will accrue a $20 administration fee.
  • Medical withdrawal refund request will be approved for the full amount with a doctor’s note being provided within 2 weeks of the refund request.
  • Refund and transfer requests made by URI Campus Recreation staff will be approved pending approval from their direct supervisor and course instructor.

Birthday/Pool Party Withdrawal/Cancellation Policy

  • All withdrawals/refunds/transfers for Learn-to-Swim will accrue a $20 administration fee.
  • No administrative fee will be administered for transfers that are instructor driven.
  • All withdrawals must be requested prior to the third class.  No refunds will be given after the third class.
  • Medical withdrawal refund amount will be based on the remaining number of classes. A doctor’s note must be provided within 2 weeks of the refund request. 
  • Double booking penalty - 50% refund with an administration fee charge of ($20) will be given for the dropped class. Refund requests must be made prior to the third class. No refunds will be given after the third class.

Hydro Fitness & Senior Water Exercise Withdrawal/Cancellation Policy

  • All withdrawals/refunds/transfers for Learn-to-Swim will accrue a $20 administration fee.
  • All withdrawals must be requested prior to the third class.  No refunds will be given after the third class.

  • Medical withdrawal refund amount will be based on the remaining number of classes. A doctor’s note must be provided within 2 weeks of the refund request.

Learn-to-Swim Withdrawal/Cancellation Policy 

  • All withdrawals/refunds/transfers for Learn-to-Swim will accrue a $20 administration fee.
  • No administrative fee will be administered for transfers that are instructor driven.
  • All withdrawals must be requested prior to the third class.  No refunds will be given after the third class.
  • Medical withdrawal refund amount will be based on the remaining number of classes. A doctor’s note must be provided within 2 weeks of the refund request.

Lifeguard Training & Review Withdrawal/Cancellation Policy

  • All withdrawals/refunds/transfers will require an administration fee ($30) per transaction 

  • If withdrawn 3 weeks prior to the start of class a full refund will be issued with the charge of an  administration fee ($30).

  • If withdrawn within 3 weeks up to the class date a 50% refund will be issued with a charge of an administration fee ($30).

  • If the registrant fails the precourse a 50% refund will be issued with a charge of an administration fee ($30). 

  • Medical withdrawal refunds will be based on the total number of the remaining classes.  A doctor’s note must be provided within 2 weeks of the refund request. 

  • Double booking penalty - If withdrawn within 3 weeks up to the class date a 50% refund will be issued with a charge of an administration fee ($30).  After the start of the class date no refund will be given. 


Private Swim Lessons Withdrawal/Cancellation Policy

  • All withdrawals/refunds/transfers for Private Swim Lessons will accrue a $10 administration fee.
  • Medical withdrawal refund amount will be based on the remaining number of classes. A doctor’s note must be provided within 2 weeks of the refund request. 

Water Safety Instructor Withdrawal/Cancellation Policy

  • All withdrawals/refunds/transfers will require an administration fee ($30) per transaction 

  • If withdrawn 3 weeks prior to the start of class a full refund will be issued with the charge of an  administration fee ($30).

  • If withdrawn within 3 weeks up to the class date a 50% refund will be issued with a charge of an administration fee ($30).

  • If the registrant fails the precourse a 50% refund will be issued with a charge of an administration fee ($30). 

  • Medical withdrawal refunds will be based on the total number of the remaining classes.  A doctor’s note must be provided within 2 weeks of the refund request.

Note: Classes are subject to cancellation if they fail to meet the minimum enrollment standard. Participants will be notified if class minimum enrollment is not met. If a replacement class is not available a full refund will be issued.


Fitness & Wellness

Sailing

Are you submitting a request to transfer from one program offering into another OR Cancel your current program/service registration?
  • No refund will be provided for Sailing and Kayak Memberships, one week after day of purchase.

  • Refunds requested within first week of purchase, without use of service, can be processed for a refund. A $25 administration fee will apply, unless waived due to extraordinary circumstances. 
  • All cancellation requests must be made by submitting a Campus Recreation Refund Request form for each participant. A $25 Administration fee will be accrued for each program cancellation if made at least two weeks prior to the program start date.   A 30% administration fee will be accrued if cancellation is made within two weeks of the scheduled start date.
  • Cancellation requests due to a change in medical status, requires submission of a Request for a Refund form and must provide a doctor's note within 2 weeks of submission and will be reviewed by the program supervisor.
